We’ll use the Langford Wellbeing Wheel when we work with you
to set, review and achieve your goals.
You’ll decide on a goal in each of the Wellbeing Domains, then we chat about what support you need from us to achieve it.
We use your NDIS goals as part of this, but we can support you to achieve other goals, too.
Here are the steps you’ll take…
You’ll meet with your Program Manager and Team Leader and talk about what goals will make your life happier, healthier and greater.
You’ll decide which Wellbeing Goals are
important to you, write them down and make a plan.
You start working towards your wellbeing goals.
We’ll share your goals with your Support Workers, so they can make sure they are supporting you to achieve your goals and keep everyone on your team up to date on your progress.
Your Team Leader and Program Manager will keep an eye on your progress and help your Support Workers understand how they can support you to achieve your goals.
You’ll meet with your Team Leader every four months to talk about your goals. You’ll talk about what’s working, what isn’t working and what your next steps are to keep your goals on track.
